Care Management
Care management is the holistic process of assessing, developing and monitoring a care plan to meet you or your loved’s one health and well-being needs. Our care managers, who specialize in gerontology, social work or nursing, are adept at planning for the best possible quality of life with the most peace of mind.
Your care manager will perform an assessment. Sometimes this means interviewing doctors to
reconstruct medical history to better understand present-day health needs. Or talking with friends and neighbors to learn about past interests so they can be brought forward as part of the care plan.
Once we have this information, we’ll develop an individualized plan that accounts for their needs, goals and interests and monitor things to ensure your loved one is getting the care they need to live as independently as possible.
